Lost Time Incident Rate is a standard OSHA metric that calculates the number of incidents that result in time away from work. Not all recordable incidents result in lost time, which is why there is a separate calculation for these more severe incidents.
Lost Time Injury rate follows a simple formula to indicate your performance. Divide the total number of lost time injuries in a certain time period by the total number of hours worked in that period, then multiply by 200,000 to get the LTIR. This number tells you the number of lost time injuries per 100 worker years (equivalent to 2000 hours worked for each of 100 workers).
Note, some companies in certain countries use 1,000,000 instead of 200,000 in the formula described above. The reason for using such large numbers is the number of lost time injuries compared to total number of hours worked is always small. Using a larger number as a baseline makes it easier to interpret. The Managing the Lost Person Incident (MLPI) course is designed to teach the essential knowledge and skills necessary to perform as the initial response Incident Commander on a lost person incident with the primary focus on the Search element of SAR. The course prepares the student for the management responsibilities and leadership skills required during SAR missions. The course explains the tasks and procedures needed to effectively manage SAR operations from the initial response through the expansion phases of the search to the final demobilization of all resources. This is the first true management course of the NASAR series of SAR courses. The ISAR, FUNSAR, and ADSAR courses focus on the individual and crew responsibilities as well as techniques and tactics at the crew level, whereas, the MLPI course explains and describes in detail the overall objectives, strategies, and tactics needed for a successful outcome of SAR missions. With further incident management training and field experience; the person with this training can perform as the Incident Commander on prolonged and more complex searches.
On Tuesday, April 5th, 2022, starting at 7:38 UTC, 775 Atlassian customers lost access to their Atlassian products. The outage spanned up to 14 days for a subset of these customers, with the first set of customers being restored on April 8th and all customer sites progressively restored by April 18th.
Although this was a major incident, no customer lost more than five minutes of data. In addition, over 99.6% of our customers and users continued to use our cloud products without any disruption during the restoration activities.
As previously mentioned, the same script that deleted customer sites also deleted key customer identifiers and contact information (e.g. Cloud URL, site System Admin contacts) from our production environments. This is notable because our core systems (e.g. support, licensing, billing) all leverage the existence of a Cloud URL and site System Admin contacts as primary identifiers for security, routing, and prioritization purposes. When we lost these identifiers, we initially lost our ability to systematically identify and engage with customers.
Once at the anchor station, the personnel realized that the vessel was not dragging anchor, but that the anchor had been lost. The main engine was engaged and the vessel was able to safely make its way out of the anchorage.

Aided Records involve occurrences when a person is injured (not involving a motor vehicle), or becomes sick or lost, and service is rendered by the New York City Police Department. Aided Records can only be obtained through the mail and are NOT available at Police Headquarters. The form can be downloaded here or is also available at any police precinct, Housing Bureau public service area, or Transit Bureau district. Two copies of the form are required along with a self-addressed stamped envelope.
